Release 1 . This is the first release of this unit of competency in the TLI Transport and Logistics Training Package.

Application

This unit involves the skills and knowledge required to investigate an incident associated with railway operations in accordance with rail safety regulatory and workplace requirements.

It includes planning the investigation, collecting and analysing information, and preparing the safety incident report.

People achieving competence in this unit will need to comply with the relevant state/territory rail safety legislation, codes of practice, rail network standards and/or guidelines.

Work is performed under some supervision generally within a team environment.

No licensing, legislative or certification requirements apply to this unit at the time of publication.

Plan the investigation 

1.1 

Clarification of the incident investigation objectives is established in conjunction with relevant personnel in accordance with workplace and regulatory requirements

1.2 

Investigation timeframes and locations are established in conjunction with relevant personnel that includes the investigation, draft report, recommendations and final report

1.3 

Site parameters, operational work systems, equipment, management and site operating systems for investigating safety incidents are determined in accordance with the scope of investigation

1.4 

Required investigation resources are identified and obtained in accordance with workplace procedures

Collect and analyse information 

2.1 

Relevant available incident information is obtained and collated to facilitate a detailed and accurate incident analysis

2.2 

Interviews are conducted with relevant personnel to establish their recollection of events associated with the incident

2.3 

Information is analysed in accordance with the established investigation objectives, based on approved conditions, applicable rail network standards and operational guidelines

2.4 

Analysis criteria are identified to enable evaluation of the findings in accordance with the scope of investigation

2.5 

Options that lead to recommendations that reduce future risk are identified and corrective actions generated in accordance with workplace and/or statutory requirements

Summarise and report on progress of investigation

Ongoing monitoring of the investigation, changes in direction and resource implications are recorded and reported

Progress, problems and resource usage are reviewed in accordance with the investigation scope

Progress reports and/or briefings are prepared and disseminated in accordance with workplace and/or statutory requirements

Records and systems are secured and updated regularly in accordance with workplace and/or statutory requirements

Contributing factors are identified and recommendations are made that will enable a satisfactory resolution of the incident issues, and meet workplace and statutory requirements

Draft incident report is distributed to relevant stakeholders for validation in accordance with workplace and/or statutory requirements

Assessment requirements

Performance Evidence

Evidence required to demonstrate competence in this unit must be relevant to and satisfy all of the requirements of the elements and performance criteria on at least two separate incidents associated with railway operations and include:

  • analysing information in accordance with agreed scope of the investigation, applicable rail network standards and operational guidelines
  • applying precautions and required actions to minimise, control or eliminate hazards that may exist when investigating safety incidents
  • applying relevant rail network instructions, procedures and information to investigating safety incidents
  • applying relevant legislation and workplace procedures
  • communicating effectively and working collaboratively with others when investigating safety incidents
  • completing documentation related to investigating rail safety incidents
  • developing recommendations and corrective actions to reduce future risk in accordance with workplace procedures
  • establishing agreed scope and methodologies of investigation with relevant personnel
  • establishing investigation timeframes and locations with relevant personnel
  • establishing the root cause and contributing factors to the safety incident
  • interpreting and following operational instructions and prioritising work
  • monitoring work activities in terms of planned schedule and scope of investigation
  • preparing draft reports and recommendations including relevant additional technical support and/or information
  • reporting on progress of rail safety incident investigation
  • working systematically with required attention to detail without injury to self or others.

Knowledge Evidence

Evidence required to demonstrate competence in this unit must be relevant to and satisfy all of the requirements of the elements and performance criteria and include knowledge of:

  • data collection and analysis techniques required when investigating safety incidents
  • documentation and reporting requirements for investigating safety incidents
  • notifiable occurrences reporting requirements prescribed by rail safety regulations
  • operational work systems, equipment, management and site operating systems for investigating safety incidents
  • problems that may occur when investigating safety incidents and appropriate actions that can be taken to resolve these problems
  • root cause analysis techniques/tools
  • scope of investigation
  • state/territory rail safety legislation, codes of practice, rail network standards and/or guidelines
  • workplace procedures and policies for the investigation of safety incidents associated with railway operations.

Assessment Conditions

Assessors must hold credentials specified within the Standards for Registered Training Organisations current at the time of assessment.

Assessment must satisfy the Principles of Assessment and Rules of Evidence and all regulatory requirements included within the Standards for Registered Training Organisations current at the time of assessment.

Assessment must occur in workplace operational situations where it is appropriate to do so; where this is not appropriate, assessment must occur in simulated workplace operational situations that replicate workplace conditions.

Assessment processes and techniques must be appropriate to the language, literacy and numeracy requirements of the work being performed and the needs of the candidate.

Resources for assessment must include access to:

  • a range of relevant exercises, case studies and/or other simulations
  • applicable documentation including workplace procedures, safety management system, regulations, codes of practice and operation manuals
  • relevant and appropriate materials, tools, equipment and personal protective equipment (PPE) currently used in industry.
